We are excited to be able to offer our patients the option of requesting appointments online from the comfort of their home.
Please note that our system allows you to request a time slot, but the appointment will not be set until you receive the ‘confirmation’ email from us. We are working as hard as we can to get you in at your preferred time, but please understand that we are also booking patients over-the-phone and in person and sometimes requested times will need to be modified. Be on the lookout for the ‘confirmed’ or ‘denied’ email after your request, and check your ‘spam’ folder just in case!
If no insurance information is provided, it will be assumed that you plan on paying out-of-pocket for your examination.
